Fall under the Map in Act 5: Desperation at the Train Wreck

Enter Co-Op Mode on Act 5
Go to the Middle Section of the Train that Falls Off
Have the Berserker Down You
Now have your partner Press the Button and you will be in Mid Air for a split second
You will Fall through the tracks and will be under the world
